The process varies a lot from week to week, but it often starts with some kind of hook like a melody or vocal line that I'll then build out. I rarely start with vocals just because I'm not used to it. If I don't have any ideas I'll just start by building out some kind of atmosphere, ambience or chord progression and see what comes to mind from there.

I'd say that I probably spend anywhere from 4-8 hours on average. I often don't have much more time than that because I'm travelling around a lot this year. And yep, I always start from scratch! I'll sometimes go through 3-4 ideas before settling on one that I'm happy with for the week, but other times I just go with the first thing I come up with.
